Job Summary:
Brunswick Street Supportive Housing project provides 24/7, wrap-around support to 38 tenants who are 50+ to age in place. A multidisciplinary team of social workers, nurses, occupational therapist, physical therapist and continuing care assistants ensure each tenant has a support plan that meets the physical health, mental health and social needs. In addition, the team has housing support workers 24/7 to support tenants in maintain their tenancies and accessing services
The Physiotherapist (PT) is part of the Brunswick Street and NECHC collaborative primary health teams. The PT will work collaboratively with a team of physicians, registered nurses, nurse practitioners, social workers, dieticians, nurses, occupational therapists and non-health professional staff, they will also work alongside an additional 0.5 FTE clinic physiotherapist.
They will offer individual and if warranted, group physiotherapy to individuals who access one or more of the following services at the NECHC: NECHC primary clinic, MOSH, Brunswick Street, and Overlook. In addition to collaboration with staff and volunteers at the NECHC, collaboration with community agencies and systems (health, justice, and income assistance) will be required. Location of work largely at 2445 Brunswick Street and NECHC primary care clinic (2131 Gottingen Street) with some need for home visits or visits to other community sites.
Education and Experience:
- Bachelor’s degree in Physiotherapy or acceptable equivalent from a recognized school of Physiotherapy
- Current registration with the Nova Scotia Regulator of Physiotherapy and eligibility for membership with the Canadian Physiotherapy Association.
- Minimum of 3 years’ experience in a similar role that requires a broad scope of practice, multiple demands and teamwork.
- Experience working with NECHC’s priority populations including 2 Spirit, Gender Diverse and Transgender (2SGDT+), African Nova Scotian (ANS), and Indigenous communities.
- A comprehensive understanding of how racism, colonization and discrimination influences access to physical and mental health care, housing and appropriate support.
- Experience working from a harm-reductive, trauma-informed and culturally appropriate lens.
- Experience in not for profits and health care setting preferred but not essential.
- Experience working in a collaborative practice setting.
- Experience working with people experiencing poverty, homelessness, and other social determinants of health issues an asset.
- Experience working in a diverse and multicultural environment.
- Valid class 5 NS driver’s license with clean driver’s abstract and access to own reliable vehicle (not for client transportation) asset.
Position Responsibilities:
- The following duties reflect the broad scope of responsibilities but are not necessarily all-inclusive:
- Complete comprehensive biopsychosocial and functional assessments based on the complexities of complex health and social issues like racism and poverty.
- Support client-led goals using evidence-based and experiential knowledge, through a harm reduction and trauma-informed care lens. Support team-initiated goals where issues of safety or other concern arise, again from a harm reduction and trauma-informed lens.
- Participate in regular capacity building and professional development training specifically around decolonialization, anti-black racism, and gender affirming care.
- Participate in and/or construct community outreach programs to expand PT access to underrepresented communities (specifically with our priority communities; ANS, 2SGDT, and Indigenous communities).
- Strong collaboration will occur with the individual, health team, and anyone else the individual identifies to implement a plan of care and monitor progress.
- May involve planning group interventions. Attention will be paid to assessing, planning, and delivering group interventions in a manner that reflects the learning needs of the community.
- To create a collaborative environment in which information, knowledge, and resources are shared among community service, health care, academic, and government organizations.
- Document on electronic medical records and participate in evaluation activities as directed.
- Complete non-PT specific tasks related to navigating health and socially complex systems with clients who have multiple barriers to care (e.g. picking up mobility equipment if clients cannot do so, advocating for funding).
